2. Matric/FSc Detailed Mark Sheets/Certificates
- Attested copies of academic records from Matriculation and Intermediate levels.
3. Domicile

4. 5 Years Professional DMC/MBBS Transcript
- Detailed Marks Certificate and MBBS transcript covering the five-year medical degree program.
5. Valid PMDC Registration
- Proof of current registration with the Pakistan Medical and Dental Council (PMDC).
6. House Job Certificate
- Certificate of completion of the house job from a recognized institution.
7. Experience Certificate
- Documents confirming relevant experience in the medical field.
8. CNIC
- Copy of the Computerized National Identity Card.
9. Duplicate Publications
- Copies of any publications, if applicable (International/ Pak Journal).
10. FCPS Part-I Result
- Certified results of the FCPS Part-I examination.
- RS.500 Bank Draft
- A bank draft in the name of Commandant CMH Lahore as a processing fee for the application.
Important Notes:
- Candidates with foreign qualifications must submit equivalency certificates issued by HEC/PMDC/PMC.
- Incomplete or late applications will not be accepted.
